Your first visit to Maxus Dental is designed to familiarize you with our dental practice and introduce you to our compassionate, caring team of dental professionals. - Arrive 15 minutes prior to appointment time
- Bring your insurance card
- Bring your drivers license
- Bring a form of payment
- Don’t forget your beautiful smile 🙂
Please take a moment prior to your scheduled appointment to print and complete our new patient form and bring it with you to your appointment so we may better assist you in a timely manner. If you would prefer, you may fax the forms to our office at 503-334-0930.
